UTD申请表：Complete the UT Dallas graduate application
申请费：$50 application fee
毕业证及文凭证：Diplomas and/or degree certificate
3封推荐信：Three letters of recommendation

（1）以上申请材料邮寄地址- Office of Admission and Enrollment Services The University of Texas at Dallas 800 W. Campbell Rd. Richardson, TX 75080-3021
（2）成绩单、毕业证和学位证不要求也不接受外部认证机构的认证，校方会提供成绩单评估服务，但需要评估费用$50
